Hand grip strength is an inevitable component in the evaluation of rheumatoid arthritis, neuromuscular, preoperative, post operative patients and community dwelling older adults’ functional capacity. Hand grip varies greatly with age, gender and the anthropometric measures when measured by hand dynamometer. The influence of above variables on hand grip when measured by modified sphygmomanometer is unknown. Further, the prediction of hand grip from age and anthropometric traits is unknown. 229 subjects (115 males and 114 females) with age 23 ± 2 and 21 ± 2 respectively were included in the study after informed c nsent. Weight and height were obtained using standard techniques. Hand grip was measured using a modified sphygmomanometer. Information regarding physical activity and health status was obtained by interview, clinical screening and stratified. Stepwise multiple regression analysis was sought out for any influence of age, height, weight and Body Mass Index (BMI) on hand grip strength. Grip strength correlated moderate to high with age (r = 0.44, p = 0.00), height (r = 0.57, p = 0.00), weight (r = 0.57, p = 0.00) and BMI (r = 0.29, p = 0.00). The regression model for handgrip strength is Hand grip = -1790.54 + 4.93557 × Age-11.7429 × Weight + 1083.4 × Height + 34.194 × BMI. Age, height and weight are the i portant determinants of the handgrip evaluation. In clinical setting, the influence of age and anthropo etric traits on handgrip shall be borne in mind when measuring handgrip by modified sphygmomanometer in age group of 20-25 year patients.

The Effect of Exercise Training on Biologic Vascular Age in Healthy Seniors.
Am J Physiol Heart Circ Physiol. 2012 Jan 20;
Shibata S, Levine BD
Arteriosclerosis with aging leads to central arterial stiffening in humans, which could be a prime cause for increased cardiac afterload in the elderly. The purpose of the present study was to assess the effects of one year of progressive exercise training on central aortic compliance and left ventricular afterload in sedentary healthy elderly volunteers. Ten healthy sedentary seniors and 11 Masters athletes (>65 yrs) were recruited. The sedentary seniors underwent one year of progressive exercise training so that at the end of the year, they were exercising ~200 min/week. Central aortic compliance was assessed by the Modelflow aortic age, which reflects the intrinsic structural components of aortic compliance. Cardiac afterload was assessed by effective arterial elastance (Ea) with its contributors of peripheral vascular resistance (PVR) and systemic arterial compliance (SAC). After exercise training, Ea, PVR, and SAC were improved in sedentary seniors and became comparable with those of Masters athletes though the Modelflow aortic age was not changed. Moreover, after exercise training, when stroke volume was restored with lower body negative pressure back to pre-training levels, the exercise training induced improvements in Ea, PVR, and SAC were eliminated. Aortic stiffening with aging was not improved even after one year of progressive endurance exercise training in the previously sedentary elderly, while left ventricular afterload was reduced. This reduced afterload after exercise training appeared to be attributable to cardiovascular functional modulation to an increase in stroke volume rather than to intrinsic structural changes in the arterial wall.

The aim of this study was to evaluate the impact of obesity and overweight on diabetes mellitus (DM) and hypertension (HTN) control in a healthy lifestyle intervention program in Iran.Within the framework of the Isfahan Healthy Heart Program (IHHP), a community trial that was conducted to prevent and control cardiovascular disease and its risk factors, two intervention counties (Isfahan and Najafabad) and one reference county (Arak) were selected. Demographic information, medical history, anti-diabetic and anti-hypertensive medications use were asked by trained interviewers in addition to physical examination and laboratory tests for 12514 adults aged more than 19 years in 2001 and were repeated for 9572 adults in 2007.In women, the frequency of HTN control change significantly neither in normal weight nor in those with high body mass index (BMI), waist circumference (WC) or waist to hip ratio (WHR). In men, the frequency of HTN control was only significant among those with high WHR, whereas the interaction between changes in intervention compared to reference area from 2001 to 2007 was significant in men with normal or high WC or WHR. In intervention area, the number of women with high BMI who controlled their DM increased significantly from 2001 to 2007 (p = 0.008), however, this figure decreased in men. In reference area, obesity indices had no significant association with DM control. The percentage of diabetic subjects with high WC who controlled their DM decreased non-significantly in intervention area compared to reference area in 2007. A non-significant increase in controlled DM among men and women with high WHR was observed between intervention and reference areas.Our lifestyle interventions did not show any improving effect on HTN or DM control among obese subjects based on different obesity indices. Other lifestyle intervention strategies are suggested.
